Remember to Provide EMMC With Your Results

Visits to facilities such as Emergency Rooms or diagnostic units outside EMMC offices for medical evaluation are often needed. At times, EMMC has difficulty getting results from independent services only because their concern is solely on your immediate situation. This often frustrates EMMC’s need to know data results about our patients.

It is important that EMMC is informed of these third party results, including the summary of the tests and instructions discussed with the patient. Additionally, it is important that EMMC receive this information in a timely manner. Often there is significant delay when immediate follow-up is crucial.

We need copies of all labs, and subsequent cultures results if obtained; final x-ray reports, all imaging, EKGs, and copy of any discharge summary.

We ask you to provide third-party providers with this note from EMMC so that all parties are aware of our requirements. Your signature below mine is your acknowledgement that you wish to have these data faxed to us. Our FAX number is provided below. Electronic imaging or any information sent on the Internet to office@endocrinemetabolic.com is also excellent.
